Please understand the following will be considered:

• Original Receipt is presented
• Products cannot be opened, worn against the skin, used, soiled, tampered with or damaged in any way
• The item is not perishable or disposable in nature
• Attempts have been made to provide another product to meet the need. An exception is available in the event that there has been a documented change in the patient’s clinical need.
